After you have requested an appointment, I will contact you to set up our 15-minute phone consultation. We will discuss your challenges and goals, and also if we are right for each other.

If we proceed, I will then let you know which forms you will need to complete and how to set up the start of therapy.

I use an online portal (Simple Practice) for billing, appointments and other intake documents. I will provide a link to the online portal once we begin our work together. 

If you do not have access to a computer, or if you are not familiar with the technology, please let me know so we can make arrangements to complete paper copies of the forms.
